"A health care reporter required to file a Mandatory Report shall maintain a current listing
with the Agency of the name, title, phone number and e-mail address of at least two individuals designated as
the contact of record for purposes of all reports filed with the Agncy and shall designate at least one such
contact person as the primary contact in each report that is filed. The failure to maintain a current
contact listing shall not constitute grouns for the waiver of any penalties imposed under this rule."

of the mandatory report, a form can be found on the Forms page.

Pursuant to Ala. Admin Code r. 410-1-3-.09, Electronic Filing, all official filings, including facility data reports,
must be submitted electronically to data.submit@shpda.alabama.gov
for compliance with CON rules and regulations.
Beginning in January 2026, an annual report filing fee, established by the Alabama State Health Planning and Development
Agency (SHPDA), must be submitted at the time of filing to meet compliance requirements. Filing fees are determined based on your facility’s
Rural ($750.00 annual reporting fee) or Non-Rural ($1,500.00 annual reporting fee) classification. Please use the map below to locate your county and verify your applicable filing fee.

All providers are responsible for reviewing the map and ensuring that the correct filing fee is submitted with their annual report.
Failure to remit the appropriate fee may result in delays in processing, additional administrative follow-up, or late fees. If you have questions regarding
